Consultation request: Submitting this form is a request for a free phone consultation and does not guarantee that an appointment or consultation will be available. A consultation is scheduled only after it has been confirmed by the practice.
Purpose of the consultation: The free phone consultation is intended to provide an opportunity to briefly discuss your needs, learn more about the services offered, determine whether the practice may be an appropriate fit, and discuss potential next steps. The consultation is not a psychotherapy session and is not intended to provide diagnosis, treatment, or a comprehensive clinical assessment.
No therapist-client relationship: Submitting this form or participating in a free consultation does not, by itself, establish a therapist-client relationship. A therapeutic relationship is established only after the practice and prospective client have mutually agreed to begin services and completed the required intake and consent processes.
Privacy: Please do not include detailed information about your mental health history, trauma history, diagnoses, medications, or other sensitive or confidential information in this form. Information submitted electronically through a website may be subject to privacy and security risks.
Communication: By providing my phone number and email address, I authorize the practice to contact me regarding my consultation request, scheduling, and related administrative matters. I understand that email, text messaging, and other electronic communications may carry privacy and security risks.

Telehealth services: If services are initiated through telehealth, additional informed consent and practice policies will apply. Telehealth services are subject to applicable Florida laws, professional standards, and practice policies.
Location for telehealth: Clients participating in telehealth sessions must be physically located in a jurisdiction where the clinician is authorized to provide services at the time of the session.
Appropriateness of services: The practice reserves the right to determine whether its services are clinically appropriate for a prospective client. When appropriate and feasible, referrals to other providers or resources may be offered.
